Chat moderation ladder
Warn, mute, escalate: which step to use and how long each punishment lasts.

1st offence: formal warning with the rule name in the reason.
2nd offence: 10 minute mute.
3rd offence: 1 hour mute, plus a staff note about the pattern.
Anything past that: escalate to a Moderator in #staff-chat with the history attached.
