When to temp-ban or perm-ban, what evidence to attach, and how appeals are reviewed.

Temp-ban for proven grief or a first cheating offence.
Perm-ban for hacked clients, repeat offences or real-world threats.
Post the ban in #staff-chat and tag an Admin for perm-bans.
